The Klamath Tribes Member Benefits Department is preparing for the 2015 Revenue Allocation, which is scheduled for Friday, June 19, 2015. Please note the allocation is based upon the amount of excess net revenues derived from the tribal gaming operation. You can come in and pick up your per capita check between the hours of 8:00 am through 5:00 p.m. Please bring proper identification with you. It is not in our policy to allow anyone else to pick up your check. If you do not come in to pick up your check it will be mailed to the address we have on file for you on Monday, June 22, 2015. Often checks will be returned to our office marked, “Address Unknown, No such Number, and Unable to Forward.” Please allow fourteen postal days prior to seeking lost checks. If you do not receive your check after fourteen days, please call, it may have been returned and or lost. After 14 days you may request a Lost or Stolen Check Affidavit. ALL checks will be VOID after 180 days and non-negotiable after December 31, 2015 per KTRAP 50.14(l).


The enclosed Address Update form must be submitted to the Member Benefits Department before May 15, 2015.  Also, don’t forget to update your minor children. Please fill out and return the enclosed “Address Update” form so we may update your record for future distributions, notices, or Klamath Tribal mail outs.


It is the responsibility of each member to keep The Klamath Tribes Member Benefits Department informed regarding your and your minor children’s address changes. If you have had a name change we require copies of name change documents, marriage license, divorce papers, adoption records in order to change your name on file. If there has been a change in guardianship of a minor child we will need copies of guardianship papers or custody awards.  In order for the Klamath Tribes to be found in compliance with the IRS we need a copy of your social security card with your correct legal name. We also require copies of wills or petitions of personal representation and death certificates to claim benefits for any member who may have passed away since the last per capita.
